Your complete health history.
One place.
You control who sees it.

Diagnoses, prescriptions, lab reports, and conversations with your doctors — organized in one timeline. Doctors see it only while you say so.

Every doctor is license-verified. Every access is logged.

How it works

Three steps. No hospital paperwork, no faxes, no lost files.

01

Create your health record

Sign up in a minute. Add past diagnoses, prescriptions, and reports — or start empty and let it grow.

02

Share a code, not a folder

Every doctor has a short code like AKT-4F7B. Enter it, they approve, and your histories connect — no paperwork.

03

Your doctors keep it current

Diagnoses, prescriptions, and advice land in your timeline as care happens. Ask questions by text or voice note.

Access you can see. And end.

Your dashboard always answers one question first: who can see my records right now?

Granting access takes a code. Revoking it takes one click, and it works immediately — no phone calls, no forms, no waiting.

Underneath, every single access and change is written to a permanent audit log. Nothing is ever silently edited or deleted; your history stays whole.

FOR DOCTORS

The patient’s whole story, before they sit down.

A patient shows you their code, you request access, they approve — and their complete timeline is in front of you: past diagnoses, current medicines, lab results. Write prescriptions with structured items, answer follow-ups by voice note, and everything you add is attributed to you, permanently.

Register as a doctorLicense verification required before patient access.

Common questions

Who can see my records?+

Only you, and doctors you have explicitly linked. Your dashboard always shows the full list, and you can revoke any doctor with one click — access ends immediately.

Are the doctors real?+

Every doctor account is verified against their PMC/PMDC license by our team before they can access any patient. Verified doctors carry a visible badge.

What happens to my data if I revoke a doctor?+

They lose access instantly. Nothing they wrote disappears from your history — your record stays complete, and every access ever made stays in the audit log.

Is Akteora free?+

Creating and owning your health record is free for patients during our pilot. Pricing for clinics comes later — your record stays yours either way.

Start with your own record.

It takes a minute, it’s free, and it’s yours — wherever your care takes you next.
